What does a Lead Practitioner do?
Lead Practitioners, also known as Key Workers, support families by coordinating care, simplifying communication between professionals, and ensuring children receive consistent, connected support that fits their everyday life.
At GROW Paediatrics, our Lead Practitioners support children from birth to 8 years by:
- Being the main point of contact for families
- Coordinating all therapists, services and supports involved in a child’s care
- Helping families understand reports, plans and recommendations
- Providing developmental support, guidance and practical strategies
- Ensuring therapy is aligned, consistent and meaningful across home, school and community
Everything we do is centred around building strong relationships, easing overwhelm and making sure families feel supported—not just in sessions, but in all the moments in between.
Our Approach
Our Lead Practitioners work side-by-side with families, taking the time to understand your child’s strengths, needs, routines and goals. They bring together the right team members (speech pathologists, OTs, nurses, psychologists, teachers and medical professionals) so that everyone is working toward the same outcomes.

- Coordinated, streamlined care that reduces stress and confusion
- Practical, everyday strategies you can use at home
- Supporting you to navigate NDIS, reports and communication between services
- Ensuring therapy is consistent, connected and achievable
Lead Practitioners bring clarity, structure and confidence to families who are navigating complex care or multiple supports.
Who we help
Children who are
- Receiving support from multiple therapists or providers
- Neurodivergent or have complex developmental needs
- In need of coordinated, team-based care
Children with
- Developmental delays or disabilities
- Complex medical needs requiring multiple services
- Communication, sensory or behavioural challenges
- Care plans or NDIS goals that involve several providers
What to expect
When you work with a Lead Practitioner at GROW Paediatrics, you can expect a supportive, family-focused partnership. We begin by understanding what matters most to you—your goals, your routine, and what feels manageable.
Your Lead Practitioner will:
- Be your primary contact for all communication
- Coordinate across your GROW therapy team and external providers
- Help you understand and implement therapy recommendations
- Attend case conferences, school meetings and care planning sessions (where appropriate)
- Adjust goals and supports as your child grows
